Need the official QHIP order form right now?

The Immunisation Service Provider order form is published by Queensland Health. Complete it electronically and email it to QHIP-ADMIN@health.qld.gov.au. Only emailed orders are accepted. If your practice has changed name, hours or location and QHIP does not know yet, contact them before you order.

We check Queensland Health's immunisation page every day, so this link always points at the current version of the form.

Ordering vaccines by hand, versus ordering with your stock already counted

Every practice nurse in Queensland knows the first list. The second one is what the same job looks like when the fridge is already in the system.

The manual way

  • ✕Open the fridge and count every vaccine, batch by batch, before you can even start.
  • ✕Work out how many doses to order for each line from memory, a notebook or last fortnight's form.
  • ✕Type each vaccine into the PDF one at a time, and count every flu dose on hand even when you are not ordering flu.
  • ✕Remember which vaccines are interchangeable and combine them in the right cell.
  • ✕Check you have the current version. Queensland Health replaced the form twice in 2026, in July and again in September, without changing the download link.
  • ✕Re-key the lot again next fortnight.

What changed on the QHIP order form in 2026

Queensland Health has replaced the Immunisation Service Provider order form twice this year. Both times the download link stayed the same and only the PDF behind it changed, so a saved copy or a bookmarked link does not tell you the form is out of date. If you send an old version, QHIP may bounce it.

Clinics holding residual stock of a removed vaccine can still record doses given and discards in ImmunaStock. The vaccine simply no longer appears on the order form.

Questions nurses ask about QHIP ordering

No. QHIP only accepts orders emailed to QHIP-ADMIN@health.qld.gov.au, so you print or save the completed form as a PDF and email it exactly as you do today. The difference is that the form arrives already filled in from your stock.

Yes. The order form inside ImmunaStock mirrors the current QHIP Immunisation Service Provider order form (v2026.09.10, September 2026). We check Queensland Health's immunisation page every day and update the app when a new version is published.

No. Every pack is scanned in on delivery and every dose is scanned out when given, so the doses on hand are already current when you open the order form. A quick scan-count at the fridge takes minutes, not hours, if you want to double check.
