Need the official QHIP discard form right now?

The Immunisation Discard or Transfer Form is published by Queensland Health. Record every batch number and the number of doses, then email the completed form to QHIP-ADMIN@health.qld.gov.au. Do not use it for vaccines left out of the fridge. That is a cold chain breach and has its own form.

Discarding expired stock by hand, versus tapping Discard

The form has not changed. What changes is that you no longer have to find the information before you can fill it in.

The manual way

  • ✕Pull the expired packs out of the fridge and read the tiny batch numbers off each one.
  • ✕Hand-write vaccine name, batch, expiry and doses for every line.
  • ✕Check the expiry rule again: if no day is printed, it expires on the last day of the month.
  • ✕Hunt for the current version of the form on the Queensland Health site.
  • ✕Update the stock register separately so the counts still match.
  • ✕Do the same again for stock transferred to your other location.

The dashboard shows Expired and Expiring Soon for every batch, using the expiry dates scanned in when the stock arrived.

Tap Discard

From the dashboard, or while scanning stock out, choose Discard. The Queensland Health form opens with that batch filled in. Add more lines if you are clearing several.

Print, email, done

Save as PDF and email it to QHIP-ADMIN@health.qld.gov.au. The discarded doses are already off your stock and in your wastage history.

Discard, transfer, or cold chain breach

Queensland Health uses one form for discards and transfers, and a different one for cold chain breaches. Sending the wrong one slows everything down.

Discard

Vaccines past their expiry date, or damaged packs. Use the Immunisation Discard or Transfer Form. Stock should not be discarded before its expiry date.

Transfer

Moving stock between practice locations. Same form, with the receiving location recorded.

Cold chain breach

Vaccines left out of the fridge, or stored or transported outside +2°C to +8°C. Do not discard them and do not use the discard form. Submit the Cold Chain Breach and Light Exposure Report Form and keep the stock refrigerated and isolated until QHIP or your local Public Health Unit tells you what to do.

Northern Territory

NT clinics report wastage through NT Health's online vaccine wastage report rather than a PDF. ImmunaStock links to it from the same Discard screen.

Questions nurses ask about discarding vaccines

No. You print or save the completed form as a PDF and email it to QHIP-ADMIN@health.qld.gov.au, as Queensland Health requires. It just arrives already filled in from your stock.

Queensland Health's instruction is that stock should not be discarded before its expiry date. If vaccines have been stored outside +2°C to +8°C, do not discard them until QHIP or your Public Health Unit advises. Report that with the cold chain breach form instead, not the discard form.

If no day is printed, the expiry is the last day of that month. ImmunaStock applies that rule when it records the batch, so the dashboard and the discard form both use the right date.
